Privacy Collection Notice

This notice explains what happens to the details you give us through the forms on this website: starting an enquiry, referring a patient, asking about the practice portal, contacting us and signing up for updates.

What We Collect

Your name and contact details, your date of birth when you start an enquiry, the treatment and its estimated cost, your practice and practitioner, and anything you write to us.

Why We Collect It

To assess your enquiry and contact you about it. With your agreement, we also contact your treating practice about your treatment plan and quote.

Where It Is Kept

In Super for Health’s own secure system, hosted in Sydney, Australia. Only the Super for Health team can see it.

Who Else Sees It

Your treating practice, only when you agree. Nothing is sent to the Australian Taxation Office until a Case Manager has checked everything with you and you choose to apply.

Searching For Your Practice

When you type your practice’s name, the words you type are sent to Google Maps to suggest matching practices. Nothing else about you is sent to Google.

You can ask to see or correct your details, or ask us to stop contacting you, at any time. Call us on 1800 951 859 during business hours.
